We are striving to have this 'intake form' be filled out 2 to 3 years prior to your target b mitzvah date.
(But if you are here later than that, no problem!) The information you provide here will help us craft this special day with you!

Prior to submitting this form, please review OZ's b mitzvah webpage which includes a b mitzvah guide and resources.

For 2023 submissions: Forms submitted by July 7, 2023 will go into the queue for dates to be assigned by August 7, 2023.

For 2024 submissions (and beyond): Forms submitted by March 1 each year will go into the queue for dates to be assigned by April 1, for the year that is two years after.

If there are off-cycle submissions, we'll do our best to assign dates as quickly and fairly as possible.

Your Kiddush Gathering After Services

* Our practice is for b mitzvah families to host a community kiddush (meal) after the service. Please share your current plans.

Holding Your Simcha (Celebration / Party):

* Separate from the Kiddush, would you like to talk to OZ staff about costs and logistics of hosting a kosher catered party in the Social Hall?

OZ may use a photo and the first name of the b mitzvah to celebrate them in OZ email newsletters and social media

Financial Commitment and Contributions

A one-time b mitzvah fee is required to help support OZ. In transparency it's a small portion of the cost of expenses related to the service, AV/tech, standard instruction, study and rehearsals. For the 2024 - 2025 year, the fee is $650. Note there are additional costs including catering and the items below that fall outside of this fee. See the b mitzvah manual for more info.
