Memorial Plaque

Purchase or Reserve Memorial Plaques

Our plaques are displayed in OZ's Samuelson Saiger Sanctuary.

You can use this form:

1. To reserve plaque space for future use;
2. To request a previously reserved plaque be created; or
3. To purchase a plaque that has not been previously reserved upon the death of a loved one.

Current contribution levels are: $250 to purchase a plaque or $310 to reserve a plaque. (Reserving entitles you to a location with a 'Reserved' plaque now, and a customized plaque produced and placed there in the future.)

The price will increase soon as follows: $360 to purchase or $435 to reserve.

INSTRUCTIONS FOR ORDERING YOUR PLAQUE:

- Submit your text for your plaque below, and complete payment details on page 2.  
- Both you and our office will receive confirmation.   Review it carefully - the text you submit will be placed on your plaque.  Note; Orders take 2-3 months to deliver from approval.
- You will be notified when the plaque is hung.


As you fill out the following, note that plaques have 4 segments of text:

1: Name in Hebrew on Line 1
2: Name in English on Line 2
3: Left Side of Line 3: Date of Death in English
4: Right Side of Line 3: Date of Death in Hebrew

Please write the exact text you would like.  Click on the field to use the Hebrew keyboard.

Please write the exact name you would like on the plaque.  Engraving will be ALL IN CAPS.
Please use this format: [4 or 5 characters for Month Number for Date, 4 Digit Year]: "Jan. 1, 2006" or "July 5, 1985" or "April 7, 2002"

June, July and April may be spelled out.  
Note: Only include the name of the month without a prefix or 'bet' in front.

An extension allows us to add 24 more plaques to our memorial boards, and it allows donors to have a special plaque on the extension.  As a sponsor, your name is featured on an extention board. 
Examples: "IN MEMORY OF BARRY J. KRIKSTONE WITH GRATITUDE OHAVI ZEDEK SYNAGOGUE"
or "IN LOVING MEMORY OF DONNA JEAN HERSHBERG BY BERNARD H. HERSHBERG"
