Tree of Life

The Tree of Life at OZ offers a unique opportunity to honor, celebrate or memorialize special people or milestones

Our Tree of Life wall sculpture is on the north wall of our lobby, below the Lost Mural and near the entrance to our social hall. Each of its hundreds of brass leaves represents a contribution to our community.

How to place leaf on our growing tree:

1. Submit your donation and text for your leaf below.
2. Indicate if you will pay by check, credit card, ACH or other on page 2. Both you and our office will receive confirmation. Review it carefully - the text you submit will be placed on your leaf.
3. The leaf will be ordered once your donation is received.
4. You will be notified when the leaf is hung on the tree
